Comments:
The Licensing Inspector conducted an unannounced inspection on 4/25/19 in response to a complaint that was received in the Licensing office on 4/22/19 concerning supervision and care of the children while on field trips. Observations were conducted, staff were interviewed, and documentation was reviewed during the investigation. Based on the information gathered during the investigation, the evidence supports the complaint and is deemed to be valid. The area of non-compliance associated with the complaint is on the attached violation notice.

Complaint related: Yes
Description: Based on interviews and a review of documentation, it was determined that the licensee did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Evidence: 1. Staff #1 stated that she was supervising a group of five children ages two years old to four years old alone. 2. The record for staff #1 did not contain any documentation to demonstrate that staff #1 was Program leader qualified. 3. Staff #2 (Program Director) confirmed that the record for staff #1 did not contain documentation to demonstrate she was Program Leader qualified.

Plan of Correction: The facility responded: Staff records will be updated with documentation to show qualifications.

Description: Based on interviews and a review of documentation, it was determined that the licensee did not ensure that when the outdoor activity area is not adjacent to the center, there shall be at least two staff members on the outdoor activity area whenever one or more children are present. Evidence: 1. Based on interviews and photographs, on 4/18/19 at approximately 3:45pm, staff #1 left the facility alone with a group of five children, ages two years old to four years old, to go on a nature walk in the neighborhood located behind the facility. 2. Staff #1 confirmed that the she was alone with the five children and there were no other staff present on the nature walk on 4/18/19.

Plan of Correction: The facility responded: Two or more staff members will be present when on the outdoor activity area that is not adjacent to the center.

Description: Based on interviews and a review of documentation, it was determined that the licensee did not ensure that there shall be at least one staff member trained in first aid, cardiopulmonary resuscitation, and rescue breathing as appropriate to the age of the children in care who is on the premises during the center's hours of operation and also one person on field trips and wherever children are in care. Evidence: 1. Staff #1 was alone with five children on a field trip on 4/18/19. 2. The record for staff #1 did not contain documentation to demonstrate she had a current CPR/First aid certification. 3. Staff #2 (Program Director) confirmed that staff #1 did not have a current CPR/First aid certification.

Plan of Correction: The facility responded: CPR was scheduled for all employees who are not CPR certified.

Complaint related: Yes
Description: Based on interviews and a review of documentation, it was determined that the licensee did not ensure that there shall be a first aid kit on a field trip and whenever children are in care.

Evidence:
1. A first aid kit was not taken on the nature walk by staff #1.
2. Staff #1 confirmed that she did not take a first aid kit on the nature walk.

Plan of Correction: The facility responded: Staff were instructed and trained on carrying a first aid kit on all field trips, including nature walks.

Complaint related: Yes
Description: Based on interviews and a review of documentation, it was determined that the licensee did not ensure that the center shall make provisions for providing children on field trips with adequate water. Evidence: 1. Staff #1 took five children on a nature walk for approximately 15 minutes, but did not bring any water. 2. The temperature outside at approximately 3:51pm on 4/18/19 was 74 degrees. 3. When staff #1 was asked if she took any water with her on the nature walk, staff #1 said "no".

Plan of Correction: The facility responded: Staff were instructed and trained on carrying water on field trips, including nature walks.

Description: Based on observation and interviews, it was determined that the facility did not ensure that before leaving on a field trip, a schedule of the trip's events and locations shall be posted and visible at the center site. Evidence: 1. There was nothing posted in the center about the nature walk when staff #1 left the facility. 2. Staff #2 (Program Director) confirmed there no schedule of the trip's event and locations posted in the facility

Plan of Correction: The facility responded: A schedule of the events and location will be posted at the learning center and visible.
