The Licensing Inspector conducted an unannounced inspection on 5/7/19 in response to a complaint that was received in the Licensing office on 5/3/19 concerning supervision of the children and injury reporting. Observations were conducted, staff were interviewed, and documentation was reviewed during the investigation. Based on the information gathered during the investigation, the evidence supports the complaint and is deemed to be valid. The area of non-compliance associated with the complaint is on the attached violation notice.

Standard #: 22VAC40-185-350-E-2 Complaint related: Yes Description: Based on on observation and interviews, it was determined that the facility did not ensure that for children,16 months old to two years, one staff member for every five children is maintained at all times children are in care. Evidence: 1. The Licensing Inspector observed three staff working with 19 children in the Two year old classroom. 2. When Staff #1 was asked the ages of the children, he stated the children present were ages 23 months old to three years old. 3. Staff #1 confirmed that the facility did not maintain the required staff-to-child ratio for this grouping of children. Plan of Correction: The facility responded: A four staff was in the kitchen when the Licensing Inspector arrived helping to prepare lunch since we were short staffed. The staff returned to the classroom while the Licensing Inspector was still present in the facility, and was only going to the kitchen for short time periods. We are currently hiring more teachers so they we are able to maintain compliance with the required staff-to-child ratios.
