Comments:
An unannounced monitoring inspection was conducted today from 9:45am-1:00pm. There were 97 children (infants-5yrs.) directly supervised by 21 staff. The physical plant, 8 staff records, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on observation, the center did not complete an updated tuberculosis screening every 2 years. Evidence: Staff 1 has a tuberculosis screening which was conducted on 1/23/17.

Plan of Correction: We will send the staff member for an updated tuberculosis screening.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain complete records for each enrolled child. Evidence: Child 1 is missing two parent work phone numbers.

Plan of Correction: We will contact the parents and get the missing information.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not keep all cleaning products in a locked place suing a safe locking method. Evidence: In Room 4, there were unlocked cleaners and disinfectant wipes in an upper cabinet.

Plan of Correction: The cabinet was newly installed. We will install a new lock for the cabinet.

Standard #: 22VAC40-185-550-D
Description: Based on record review, center did not conduct a monthly practice evacuation drill. Evidence: In November 2018, January 2019, and April 2019, a monthly evacuation drill was not practiced.

Plan of Correction: We will ensure drills are conducted monthly even if other drills are conducted during that month.
