Comments:
An unannounced monitoring inspection was conducted from 2:45 - 5:30 pm with the center assistant director and owner. There were 76 children, ranging from four months to five years of age, present with 13 staff supervising. Children were observed napping, waking up and using the bathroom, eating snack and watching a video. Snack served today was rice crackers, gogurt and water. Six children's records were reviewed and in compliance. Four staff records were reviewed. The emergency escape drill log was reviewed. Description: Based on a review of four staff files, and an interview with the owner, it was determined that repeat tuberculosis (TB) test/screenings were not completed every two years. Evidence: The TB test for staff C is dated: 2/25/17 and the TB test for staff D is dated: 11/12/16.

Description: Based on observation, not all hazardous substances were kept in a locked place inaccessible to children. Evidence: In the older toddler's bathroom, in an unlocked cabinet, there were two cans of suntan lotion, one bottle of hydrogen peroxide and a cleaner with bleach. In the infant room, a bleach water solution was observed on an open shelf.

Description: Based on observation, the facility failed to ensure that there was at least 12 inches of space between occupied rest mats. Evidence: in the 3/4 year old classroom, occupied rest mats were touching.

Description: Based on observation, the facility failed to follow hand washing procedures as required. Evidence: the LI observed staff changing the diapers in several classrooms. After diapering a two year old, two toddlers, and an infant, staff placed the children back on the floor without washing their hands with soap and water. Staff also failed to wash their own hands between diaper changes.

Description: Based on observation of diapering, it was determined that disposable diapers were not disposed of in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: staff were observed throwing diapers away in trash cans located outside of the building. Two staff were observed using their hands to open the door, and to open up the trash can for disposal.

Description: Based on review of three medications, the center did not have written authorization to administer medication to a child in care. Evidence: The center did not have a medication authorization form signed by a physician and parent for Child #3's medication.

Description: Based on review of the emergency response drills record, the provider did not conduct monthly emergency drills as required. Evidence: The last documented emergency escape drill was 3/17/19. There were no documented shelter-in-place drills in the past 12 months.
