Thank you for your assistance during today's unannounced focused inspection conducted from 3:10 PM to 4:55 PM to determine corrections to the violations with standards at the previous inspection. Today, there were twenty-five children, ages five through age eleven years, with two to three staff. Two licensing staff viewed program activities, daily routines, five records for children, two staff records, staff qualifications, staff training, snack and indoor areas. The children picked out a movie to view later this week. Also, they enjoyed active play in the gym during a game of "Sharks and Minnows". This program has demonstrated substantial compliance with minimum standards. Let me know if you need any assistance. 540-430-9259

Standard #: 22VAC40-185-70-A Description: Based on a review of two records for staff, and interviews with staff, there was required information missing from the on-site record for one staff. Evidence: The name, address, and telephone number of an emergency number for staff member 2 was not located at the school site. The form which asked for this information and pertinent health information was not found in the staff file. Plan of Correction: The site director immediately arranged for the staff member to write the name, address and telephone number for an emergency contact and to add any relevant health information for the record kept on site. The site director will make sure the emergency contact information and health information are kept at the school site when staff transfer from another program location.
