Comments:
A complaint was received in the licensing office on May 15, 2019 regarding care and protection of children. On May 20, 2019, an on-site investigation was conducted. Classrooms were observed and interviews conducted with staff. There was a preponderance of evidence to deem the complaint valid. Unrelated violations were also cited today. Standard #: 22VAC40-185-340-A
Complaint related: No
Description: Based on interview with staff, when supervising children, the staff did not ensure the care, protection and guidance. Evidence: Staff did not immediately intervene when a child was tackled to the ground by a group of children, and then continued to wrestle with each other before staff stopped it.

Plan of Correction: During our class meeting on 5/20/19, we reviewed procedures for the transition from center time to lunch time. One teacher will remain with the children to supervise interactions as well as assist with clean up.

Complaint related: No
Description: Based on observation, not all daily activities are not age and stage appropriate. Evidence: Children were sitting on the rug listening to a book from 11:5am-12:08pm, most children had lost interest and had stopped paying attention. During this time children were getting up to wash their hands and sit at the lunch table. Food was served waiting for all the children to wash their hands and told "not to touch their food", some children had been sitting there for over 20 minutes.

Plan of Correction: During our class meeting on 5/20/19, we reviewed procedures for the transition from center time to lunch time. The teacher who is conducting circle will change activities when then children lose interest. Also, the children will not be served lunch until all children are seated.

Description: Based on observation, staff did not sit with children during lunch time. Evidence: Staff did not remained seated with children during lunch time, one staff member sat for a few minutes , while the other 3 staff members stood.

Plan of Correction: During our class meeting on 5/20/19, we reviewed lunch time procedures and retrained staff that they at to remain seated with children during lunch. One teacher at a time can get up to do seconds on food. We will review this with all staff at our staff meeting on 5/29/19
