63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted today. There were 24 children in care ages 2 to 5 years old. There were five staff present. Children and staff records were reviewed as well as required postings and emergency drills. During the inspection children were resting and napping. Violations regarding staff records, physical plant and background checks were identified and appear on the violation notice.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview the center failed to ensure that each staff record contains all required information. Evidence: 1 - The record for staff 2 does not contain information regarding reference checks to include the date the reference checks were obtained or the name of the firm contacted. 2 - Staff 1 confirmed that the reference check documentation for staff 2 is incomplete.

Plan of Correction: Going forward center management will ensure that all required documentation is clearly documented in each staff record.

Standard #: 22VAC40-185-270-A
Description: Based on observation and interview the center failed to ensure that equipment is maintained in safe and operable condition. Evidence: 1 - The goal post for the basketball hoop on the outside playground has areas of rust and peeling paint within reach of children. 2 - There is a balance beam on the playground that has areas of splitting and deteriorating wood. 3 - Staff 1, 2 and 3 stated that children had used the playground in the morning and will go outside after snack today.

Plan of Correction: Center management will ensure that the basketball goal post is painted and the balance beam will either be sanded and painted or removed.

Description: Based on record review and interview the center failed to ensure that staff submit to fingerprinting prior to employment. Evidence: 1 - The record for staff 2 contains a fingerprint finding dated May 16, 2019. However, the documented date of hire is May 13, 2019. Therefore, the fingerprint was obtained after staff 2 was hired. 2 - Staff 1 confirmed that the hire date written on the employment application for staff 2 is May 13, 2019 and the date of fingerprint results is three days later, May 16, 2019

Plan of Correction: Going forward center management will ensure that each staff submits to fingerprinting before the documented date of hire.
