Comments:
An unannounced monitoring inspection was conducted on May 20, 2019 from approximately 1:30 pm to 3:10 pm. Four staff were present with seventeen children. Five children's records were reviewed. Required information to maintained on site and training were reviewed for the four staff present. Please complete the columns for description of action to be taken and date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. Description: Based on review, facility did not ensure 4 of 7 injury reports have documentation of required information. Evidence: The following was found: 1. An injury report completed on May 16, 2019 did not document the date and time parent was notified of the injury and the plan for preventing reoccurrence of the injury. 2. An injury report completed on April 31, 2019 did not document the date parent was notified of the injury and the plan for preventing reoccurrence of the injury. 3. Two injury reports completed on April 17, 2019 did not document the time parent was notified of the injury.

Plan of Correction: The Center Supervisor will ensure all accident forms are current and older forms are deleted. All required information was notated on the Accident Report Log.
