Comments:
An unannounced non-mandated monitoring inspection was completed on 05/16/2019 from 10:40 A.M.-1:15 P.M. The inspection was conducted to a response of a self-reported incident that occurred at the center on 05/07/2019. One child's records, three staff records, attendance sheets, sign in and out forms were reviewed, and four staff were interviewed. The exceptions to compliance are noted on the violation notice. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interviews, staff failed to ensure the care and protection of a three year old child. Evidence: 1. On 5/07/2019 a three year old child was out of sight and sound supervision of staff for approximately ten minutes. There were eighteen children, ages two to four years of age, on the playground with four staff members. Staff #1 completed a head count at approximately 4:50 P.M. and realized a child was missing. Staff #1 completed a name to face check and determined that Child #1 was the missing child. Staff #1 notified Staff #2 that Child #1 was missing. Following a search of the playground area by Staff #1 and Staff #2, Staff #1 entered the building and continued the search. Child #1 was located in Classroom A at approximately 5:00 P.M. where the child was observed standing alone in the middle of the classroom looking around. 2. The child went into the building through the Resource Room door, and then crossed the hallway and went into Classroom #1. 3. Staff #1 verified that the staff members were not aware that a child was missing from the playground until a head count was completed, and identity of the missing child was determined following completion of a name to face check.

Plan of Correction: Staff will be retrained to perform name to face counts and head counts at regular intervals especially on the playground.

On 05/07/2019 a three year old child was out of sight and sound supervision of staff for approximately ten minutes. There were eighteen children, ages two to four years of age, on the playground with four staff members. A head count was conducted by Staff #1 at approximately 4:50 P.M. after some children were picked up. Staff #1 realized a child was missing and following completion of a name to face check, it was determined that Child #1 was the missing child. Child #1 was located in Classroom A at approximately 5:00 P.M. where the child was observed standing alone in the middle of the classroom looking around. 2. Staff #1verified that staff members were not aware that Child #1 left the playground.

Plan of Correction: Staff will be trained to position themselves by the entrances to the playground and perform regular head counts.
