An unannounced monitoring inspection was conducted today, May 9, 2019 in response to a self reported incident regarding an injury that occurred on 04/23/2019. The Department was notified of the incident on 04/23/2019. The inspection was initiated at 8:30 am and concluded at 9:15 am. Staff interviews, review of documentation and an inspection of the playground was conducted. The preponderance of the evidence gathered during the investigation supports areas of compliance with the licensing standards.
