Technical Assistance:
Discussed supervision, requirement for background checks, resilient surfacing, play areas and equipment.

Comments:
An unannoucned monitoring inspection was conducted beginning at 10:45 a.m. and ending at 1:00 p.m. There were four children in care ranging in age from three to five years of age with one staff. Children were observed on the playground, swinging on swings and then in the outdoor play area playing in the sandbox and participating in imagination play with staff. The classroom, equipment and supplies were observed. The playground and climbing structure were inspected. Three children's records and one staff record were reviewed. Emergency drills were reviewed and up to date. There is no medication on site. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, not all equipment of the center was maintained in safe condition. Evidence: The middle green slide on the climbing structure has a two inch crack in the composite material at the top/seat position of the slide that creates pinching and scratching hazards. On the backside of the climbing structure, there are three screws on the right side of the ladder that are exposed more than two thread lengths that cause scratching and impalement hazards.

Plan of Correction: The climbing structure was placed off limits during the inspection. It will not be used until any hazards have been remedied.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center failed to ensure there was resilient surfacing in fall and use zones under play equipment. Evidence: There is no resilient surfacing for the green climbing play structure that has three swings, two slides, and a climbing wall. Dirt, grass and weeds were observed under and around the equipment.

Plan of Correction: The climbing structure was placed off limits during the inspection. It will not be used until resilient surfacing is sufficient in fall and use zones.
