Comments:
An unannounced monitoring inspection was conducted today. There were 7 7 children in care with 14 staff. Children present ranged in age from five months to five years old. Children and staff records were reviewed. Children were observed in teacher and self-directed activities including transitions, play, and nap. Required postings, emergency evacuations, emergency items, medication/diaper cream, and annual inspections were reviewed. The following violations were identified during the inspection include physical plant and unlocked chemicals and appear on the violation notice. These violations were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and interview the center failed to ensure that areas are maintained in safe condition. Evidence: 1 - The emergency exit in the "blue" room is blocked by toys and equipment. 2 - There is extension cord stretched out along a wall in the "yellow" room which creates both a trip and entanglement hazard. 3 - There is a box fan in use in the playroom within reach of children. The grate on the fan has spaces large enough for an adults fingers to fit through and therefore a child's fingers could fit inside. 4 - Staff 1 confirmed that the emergency exit is blocked, the extension cord is on the floor and the box fan is in use.

Plan of Correction: Corrected during the inspection: 1 - Toys removed from blocking exit 2 - Cord removed from room. 3 - Fan removed from classroom. Center management will ensure that these items are not an issue in the future by informing and training all staff of the corrections.

Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in locked location. Evidence: 1 - There a spray can of air freshener on an open shelf in the bathroom between the "blue and green" rooms. 2 - There is a pump bottle of hand sanitizer in an unlocked cabinet in the "green" room. 3 - Staff 1 confirmed that the chemicals were in unlocked locations

Plan of Correction: Corrected during the inspection. All staff will be reminded and retrained in the need for all chemicals to be stored in locked locations when they are not in use.
