Comments:
An unannounced inspection was conducted on April 9, 2019 from 11:15 a.m. to 12:45 p.m. to investigate a self-reported incident regarding supervision. Staff interviews were conducted. Documentation including written staff statements of the incident were reviewed. Please provide a plan of correction for the noncompliance no later than close of business, May 22, 2019. Violations:
Standard #: 22VAC40-185-340-F
Description: Based on review of documentation and interviews with staff, it was determined that staff failed to ensure that children under the age of 10 years of age were within actual sight and sound supervision of staff at all times. Evidence: On April 3, 2019, at approximately 4:10 p.m. Child A, age 4 and Child B, age 5 were unsupervised in the bathroom on the playground. A third child reported that Child A and Child B were in the bathroom together to staff at approximately 4:17 p.m. According to staff interviews and written statements, no staff observed the children entering the bathroom.

Plan of Correction: Both teachers were retrained on playground safety and supervision on April 10th 2019. A door alarm was installed on the playground bathroom door on April 15th to alert everyone on the playground when someone is entering or leaving the restroom. We also had a school wide Staff Meeting on April 10th on Playground Safety and supervision.
