The licensing inspector conducted an unannounced monitoring inspection on Friday, May 24, 2019 from approximately 10:20 AM until 1:30 PM. The following ratios were observed with a total of 63 children present during the inspection: Infants - 12 children with 5 staff Toddlers - 16 children with 4 staff Preschool - 35 children with 4 staff School Age were not present during the inspection During the inspection, the children were observed playing with toys, working puzzles, sharing Show-N-Tell, having books read to them, singing songs, playing outside, having their diapers changed, washing hands, taking bathroom breaks and eating lunch. Later, children were observed resting at nap time. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The center was equipped with toys and supplies, and items were available to the children. The provider stated transportation is not provided at this time. Medication is administered and medication, authorization forms and allergy plans were reviewed. During the inspection, six children's and six staff records were reviewed. The following information was reviewed: emergency supplies, injury reports, injury prevention plan, fire, lock down and shelter-in-place drills, daily attendance, infant daily sheets, parent agreements, liability insurance policy, required postings and staff training information. Standard #: 22VAC40-185-160-A Description: Based on review of six staff records, the center did not ensure one staff member submitted documentation of a negative tuberculosis screening no later than 21 days after beginning employment. Evidence: During the inspection, it was observed the record for Staff #6 (hire date 04/15/19) contained a negative tuberculosis test dated 05/11/2019. Plan of Correction: We will make sure the tuberculosis testing or screening is obtained prior to or within the first 21 days of being hired.
