Comments:
The licensing inspector conducted an unannounced renewal inspection on 05/16/2019 from 9:10am to 2:55pm. The children were observed playing in centers, playing on the playground, and eating morning snack. Morning snack consisted of muffins and milk. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today, as well as the center's van.

Five children?s records and five employee records were reviewed during this inspection. There were no injury reports on file. The center administers medication; however they did not have any being given at this time.

The center's first aid kit and emergency supplies were inspected.

The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review, the center did not ensure that one of five children's records contained documentation of immunizations as required by the state board of health before the child can attend the center. Evidence: The record of child #5 (start date: 4/22/2019) did not have documentation of current immunizations as required by the state board of health.

Plan of Correction: We will inform the parent and will obtain the documentation no later than the 22nd of May.

Standard #: 22VAC40-185-70-A
Description: Based on review, the center did not ensure that three of five staff records had documentation of two or more references before employment. Evidence: 1. The record of staff #1 (start date: 4/24/2019) did not have documentation of the two required reference checks. 2. The record of staff #2 (start date: 1/22/2019) had documentation of one reference check. 3. The record of staff #3 (start date: 5/06/2019) had documentation of one reference check.

Plan of Correction: We will do the reference checks today.

Standard #: 22VAC40-185-320-B
Description: Based on observation, the center did not ensure that sinks with water do not exceed 120?F. Evidence: During the inspection, the inspector measured the temperature of the water from the boys bathroom hand washing sink. The water temperature was measured at 134.2?F.

Plan of Correction: The owner will fix it today.

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that there was an insufficient amount of resilient surfacing in the use zone of playground equipment. Evidence: There was approximately 1 1/2 to 5 inches of mulch in the use zone of the pink slide. The pink slide was 2 feet and 4 inches tall. There was approximately 2 inches of mulch in the use zone of the orange slide. The orange slide was 1 foot and 4 inches tall. There was approximately 2 to 3 1/4 inches of mulch in the use zone of the spirobounce. The inspector interviewed staff members who revealed that the children use the playground equipment.

Plan of Correction: We will have 8 inches of mulch by Tuesday.
