Comments:
A Subsidy Health and Safety inspection was conducted on 05/16/2019 from 9:10am to 2:55pm to review supplemental health and safety requirements. The children were observed playing in centers, playing on the playground, and eating morning snack. Morning snack consisted of muffins and milk. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today, as well as the center's van.

Five children?s records and five employee records were reviewed during this inspection. There were no injury reports on file. The center administers medication; however they did not have any being given at this time.

The center's first aid kit and emergency supplies were inspected.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-520-B-11
Description: Based on review, the vendor did not ensure that one of five children's records did not contain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record of child #3 (start date: 2/1/19) did not contain documentation of an allergy care plan.

Plan of Correction: We will provide an allergy care plan form to the parent and it will be returned by May 21st.

Standard #: 22VAC40-665-580-D
Description: Based on review, the vendor did not ensure that five of five staff records contained documentation of orientation training for staff completed on the following facility specific topics prior to the staff member working alone with children and within seven days of the date of employment or the date of subsidy vendor approval.
Evidence:
The record of staff #1 (start date: 4/24/2019), staff #2 (start date: 1/23/2019), staff #3 (start date: 5/6/2019), staff #4 (start date: 1/7/2019), and staff #5 (start date: 1/7/19) did not contain documentation of orientation training covering the following: emergency preparedness plan, shaken baby syndrome, safe sleep, and food allergies.

Plan of Correction: We will conduct orientation today.
