Comments:
An unannounced monitoring inspection was conducted on Monday, May 20, 2019 to determine the program's compliance with licensing standards. The inspector arrived at 10:15 am and concluded the inspection at 2:00 pm. The census for today?s inspection consisted of 68 children in the direct care of 11 staff. The center?s director and assistant director assisted the inspector throughout today?s inspection. During the inspection, children were observed in teacher and self-directed activities including circle time, interest center play, transitions, hand washing, restroom routines, outside time, lunch, and nap. The center is equipped with age appropriate materials and equipment for the children?s use. Staff were engaged with the children and offering guidance when needed. The areas where children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. Six children's records, five staff?s records, and four medication administration and authorizations were reviewed today. The monthly menu was posted and today?s lunch consisted of chicken nuggets, bread and butter, mixed fruit, and milk. The most recent health inspection was on 08/06/2018. The most recent fire inspection was on 10/11/2018. The last documented evacuation drill was held on 05/15/2019 and the last shelter-in-place drill was documented on 09/04/2018. Standard #: 22VAC40-185-140-B
Description: Based on a review of six children's records on 05/20/2019, the center did not ensure that if a child, two years of age through five years of age has had a physical examination prior to attendance, it shall be within 12 months prior to attendance. Evidence: 1) The physical examination in the record of Child #4 (4-year-old), whose enrollment date was 09/17/2018, is dated 08/26/2016.

Plan of Correction: Per the Center: Moving forward the administrator will ensure that all physicals will be conducted within 12 months of enrollment for 4yo children.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of five staff records on 05/20/2019, the center did not obtain the results of a national fingerprint-based criminal history record check for three staff members within the required time frames. Evidence: 1) The record for Staff #1, hired on 12/28/2018, contained the results of a national fingerprint-based criminal history record check dated 01/04/2019. 2) The record for Staff #2, hired on 11/19/2018, contained the results of a national fingerprint-based criminal history record check dated 11/20/2018. 3) The record for Staff #3, hired on 09/01/2016, contained the results of a national fingerprint-based criminal history record check dated 10/11/2018. As of 01/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check is required to be obtained prior to employment. For staff employed prior to 01/22/2018, a statement of eligibility based on a national fingerprint-based criminal history check was required to be obtained prior to 09/30/2018.

Plan of Correction: Per the Center: The director will ensure that all employees will comply with background check regulations receiving background check results prior to orientation and current staff are up to date.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of five staff records on 05/20/2019, the center did not request, within 30 days of hire, a search of out-of-state founded complaints of child abuse for a staff member who has lived outside of Virginia within the last five years. Evidence: 1) During the inspection, the Sworn Statement for Staff #1, hired on 12/28/2018, indicated the staff member had lived outside of Virginia within the last five years. The record did not contain documentation of a requested founded complaint of child abuse from the state the staff member indicated she had lived in within the last five years.

Plan of Correction: The request was sent on 05/21/2019.
