Comments:
A Subsidy Health and Safety Inspection was conducted on Monday, May 20, 2019 to review supplemental health and safety requirements. The inspector arrived at 10:15 am and concluded the inspection at 2:00 pm. The census for today?s inspection consisted of 68 children in the direct care of 11 staff. The center?s director and assistant director assisted the inspector throughout today?s inspection. During the inspection, children were observed in teacher and self-directed activities including circle time, interest center play, transitions, hand washing, restroom routines, outside time, lunch, and nap. The center is equipped with age appropriate materials and equipment for the children?s use. Staff were engaged with the children and offering guidance when needed. The areas where children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. Six children's records, five staff?s records, and four medication administration and authorizations were reviewed today. The monthly menu was posted and today?s lunch consisted of chicken nuggets, bread and butter, mixed fruit, and milk. The most recent health inspection was on 08/06/2018. The most recent fire inspection was on 10/11/2018. The last documented evacuation drill was held on 05/15/2019 and the last shelter-in-place drill was documented on 09/04/2018. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on a review of five staff records on 05/20/2019, the vendor did not ensure that one staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: 1) The record for Staff #1, hired on 12/28/2018, contained the results of a TB screening that were dated 01/08/2019.

Plan of Correction: Per the Vendor: The director will ensure all new hires receive CPR & 1st Aid training within 90 days of hire and TB documentation within 30 days of hire and prior to contact with students via checklist.

Standard #: 22VAC40-665-580-E-1
Description: Based on a review of five staff records on 05/20/2019, the vendor did not ensure that one staff obtained current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 90 days of employment. Evidence: 1) The CPR certification in the file for Staff #1, hired on 12/28/2018, was dated 04/27/2019.

Plan of Correction: Per the Vendor: The director will ensure all new hires receive CPR & 1st Aid training within 90 days of hire and TB documentation within 30 days of hire and prior to contact with students via checklist.

Description: Based on a review of five staff records on 05/20/2019, the vendor did not ensure that one staff obtained current certification in first aid within 90 days of employment. Evidence: 1) The first aid certification in the file for Staff #1, hired on 12/28/2018, was dated 04/27/2019.
