Technical Assistance:
Inspector will e-mail renewal application to owner. The Fire Marshall was called, the annual fire inspection is scheduled for 7/29/19.

Comments:
An unannounced renewal inspection was conducted from 9:30 am-11:45. There were a total of 58 children with 13 staff members present. Children were observed engaged in group activities, circle time, manipulatives and worksheets. A selection of staff and children records, medications and other documents were reviewed. The inspection was reviewed with the Infant lead teacher in the absence of the center Director. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-130-B
Description: Based on a review of children records, the center has not obtained documentation of additional immunizations once every 6 months for each child. Evidence: Child #1, date of birth 5/7/18 does not have updated immunization record. The documented immunization is 5/8/18 and 7/5/18.

Plan of Correction: We will contact the parent for updated immunization record.
