Comments:
The Fairfax Licensing office was informed of a self reported incident regarding supervision by center administration. Based on staff interviews and video documentation, the report was valid. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on video and staff statements, the center staff did not follow its own policies to ensure supervision and ratios were in compliance. Evidence: 1. Staff in the Toddler classrooms did not notify the center administration to maintain ratios on the playgrouond. The staff handbook states "it is the staff's person responsibility to notify the administration. DO NOT leave your class out of ratio" However, the toddler classroom was out of ratio when teacher #1 went inside to change diapers and left the group outside with only two teachers and 13 children. The ratio for that group should have been 1:5 and exceeded the ratio during the time the staff member was inside. According to the staff it was for "several minutes" 2. The teachers did not count the children as required for transitions to and from the classroom to the playground. The teachers should have had 14 children with them when returning to the center classrooms and it is visible on the video that only 13 are lined up and returning to the center at approximately 3:33 pm. The handbook states "Count the children before leaving your classroom, upon arriving at the playgorund, when lining up to come back inside and upon returning to the classroom!"

Plan of Correction: 1. All staff retrained o maintaining ratio at all times. Extra safety precaution was set in place, walkie talkies were provided to each class to be used by staff to communicate to administration from the playground. 2. "Playground Tracking Sheet" policy requires attendance, using each child's picture, upon departure from the playground classroom and playground.

Standard #: 22VAC40-185-340-F
Description: Based on video documentation and staff statements, a child under 10 years of age was not in sight and sound supervision for approximately 5 minutes. Evidence: The video shows Child #1 was on the playground and did not come into the center with the group at approximately 3:33 pm. The child was alone on the fenced playground and no staff member was nearby to provide immediate intervention if needed and staff were not aware the child was on the playground. Staff # 4 is seen at the moon bounce and realizes that Child #1 is still on the playground at approximately 3:38. Staff #4 retrieves the child from the playground and hands off child #1 to Staff #5 at approximately 3:39 pm.

Plan of Correction: All staff were retrained on the school's "Name to Face" and "Playground Tracking Sheet" policies. Name to Face requires hourly attendance using child's picture. "Tracking Sheet" requires attendance, using each child's picture. Upon each child's picture, upon departure from the classroom and playground.

Standard #: 22VAC40-185-350-C
Description: Based on staff statements, ratios for the two classrooms was not maintained on the playground. Evidence: Two toddler classrooms went outside to the playground with 14 children and 3 staff. The ratios were 1:5 as the youngest child was under two years of age. According to the teachers (#2 and #3), teacher #1 left the playground for approximately "several minutes" to change a diaper leaving the group out of ratio as it exceeded the 1:5 requirement (2 teachers with 13 children).

Plan of Correction: All staff retrained on maintaining ratio at all times. Extra safety precaution was set in place, walkie talkies were provided to each class to be used by staff to administration from the playground.
