Comments:
An unannounced monitoring inspection was conducted today from 12:45pm - 3:15pm. There were 56 children (2yrs-5yrs.) directly supervised by 10 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed napping and reading books. There was an abundant supply of materials for the children. Areas of non-compliance are identified on the Violation Notice. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, the center did not maintain complete records for each staff member. Evidence: Staff#2 is missing 1 verified reference as to their character and reputation, as well as competency required prior to the first day of employment, and documentation of orientation. Staff#3 is missing documentation of orientation. Staff#4 and #5 are missing 2 verified references and documentation of orientation.

Plan of Correction: We will remind teachers to bring their reference letters, and individually, I will go over the orientation paperwork with them.

Standard #: 22VAC40-185-510-C
Description: Based on review of medication, one outdated medication had not been returned to the parent to obtain a current medication. Evidence: Child#6 has a long term emergency medication on site that expired on 3/2019.

Plan of Correction: I will personally speak to the parents when they pick up, I will request a new medication from them.

Standard #: 22VAC40-185-510-N
Description: Based on review of medication and interview, it was determined that a child had two medications on site without a medication authorization form. Evidence: Child#6 has two long term medications on site. Both of these medications were observed on site without a medication authorization form completed and signed by the parent and the physician.

Plan of Correction: We will provide the forms to the parents to take to their pediatrician.
