Comments:
An unannounced monitoring visit and facility tour was conducted from 10:00am-11:45am. At the time of inspectors' arrival there were 85 infant to kindergarten aged children in care with 16 staff members and 1 administrators. Children were observed participating in outdoor play, indoor play, transitioning between activities, handwashing, nap, and educational activities. A sample of 8 children's records and 8 staff records were reviewed. Medications and injury reports were also reviewed. The areas of noncompliance are detailed in the violation notice and were discussed with the director during the exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review and interview it was determined that the center did not ensure that all staff resubmit TB test results at least every two years. Evidence: 1. The record for staff #1(hired 2/2017) did not contain updated TB test documentation. The documentation available was dated 2/10/17. 2. The Director confirmed updated documentation was not available during this inspection.

Plan of Correction: Staff will get updated TB test/screening.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: 1. On the outdoor playground there were 38 tears ranging from 2-14" in the poured in place resilient surfacing. 2. These areas present potential injury and trip hazards.

Plan of Correction: The center will repair or replace the poured in place resilient surfacing.

Standard #: 22VAC40-185-510-A
Description: Based on record review and interview it was determined the center did not ensure that medication is only given with written authorization from the parent. Evidence: 1. The medication administration consent form for child #9 did not contain the parent's signature. 2. According to the medication log medication had been administered to child #9 on 10/30/18 and 12/14/18. 3. The Director confirmed the parent had not signed the consent form in the record.

Plan of Correction: Director will have parent sign the form at pick up.

Standard #: 22VAC40-185-510-J
Description: Based on observation and interview it was determined that the center did not ensure that all medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In room #1(infants) there was a prescription bottle in the lowest drawer of a non locking plastic storage container. The prescription bottle was labeled Aspirin and Ibprofen. 2. Staff in room #1 stated it was their medication and they had just put it there today.

Plan of Correction: Medication was removed by staff during the inspection.
