Comments:
An unannounced monitoring visit was conducted today. All rooms were in ratio with direct care staff. Observed children in infant care, diapering, preparation and transition to lunch (spaghetti with meat sauce, spinach salad and fruit), reading, outside play time, room free play and rest time. Emergency medication is on-site with several staff having Medication Administration Training (MAT) certification. Emergency drills are current. Infant bottles were labeled and dates as well as topical creams (diaper cream and sunscreen). Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-160-A
Description: Based on record review, 1 staff record is missing documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment. Evidence: Staff #1 did not have documentation of a TB screening in the record and was hired 4/11/2019.

Plan of Correction: Staff #1 submitted TB test (negative) dated February 2018. Requested current TB test by 5/31/2019.

Standard #: 22VAC40-185-240-A
Description: Based on record review, 1 staff did not have documentation of receiving training by the end of the first day of assuming job responsibilities. Evidence: Staff #1 was hired 4/2019 and the orientation form was not signed that the training had been completed.

Plan of Correction: Orientation was completed on 4/11. Director failed to document. Now complete and in file.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were unlocked in a classroom. Evidence: A blue cabinet in the 3's classroom was unlocked and contained cleaning materials.

Plan of Correction: Cabinet locked 5/15/2019. All staff in 3's classroom reminded to lock all cabinets.

Standard #: 22VAC40-185-500-B
Description: Based on observation, a diapering surface is no longer non-absorbent. Evidence: The diaper pad in Infant B classroom has a small slit in the plastic covering and would not prevent liquids from seeping into the foam padding.

Plan of Correction: Changing pad replaced 5/15/2019.
