Discussed expired epi-pens and when to return them to the parents.

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 9 staff files, 10 children's files, the annual fire inspection, children's activities, supervision, staff training, medications, emergency procedures, drills and supplies were inspected between 11:50-2:00pm. There were 90 children under the supervision of 16 staff. The children were observed eating lunch, working on spelling words and counting numbers, reading books, participating in circle time, transitioning to other activities and preparing for naptime. The children bring in their lunches from home each day they are in care. The interactions between the children and staff were seen to be positive and constructive in nature. Standard #: 22VAC40-185-130-B
Description: Based on file review, 2 out of 10 children's records do not have documentation of additional immunizations once every six months for children under the age of two years. Evidence 1. Child #7 is 23 months old. The most recent immunization on file was completed when the child was 15 months. 2. Child #10 is 21 months old. The most recent immunization on file was completed when the child was 7 months.

Plan of Correction: We will ask the parents for this.

Standard #: 22VAC40-185-160-C
Description: Based on file review, 9 out of 9 staff records do not have a TB test/screening completed every two years. Evidence 1. Staff #1-9 have TB test/screenings that were most recently completed between 1/24/17 and 3/20/17.

Plan of Correction: These will be completed again.

Standard #: 22VAC40-185-60-A
Description: Based on file review, 1 out of 10 children's records does not have all required information. Evidence 1. Child #9 does not have the name, address and phone number of 2 emergency contacts in case the parents cannot be reached.

Plan of Correction: We will ask the parent for this.

Standard #: 22VAC40-185-510-N
Description: Based on observations, the center did not return a medication to the parents within 14 days of authorization or dispose of it after 14 days. Evidence 1. In classroom #3, there was a medication authorization for child #11, which expired on 2/15/19. Both the expired medication authorization and expired medication (2/19) were still being kept in the classroom.

Plan of Correction: The child is no longer attending the center, so this will be disposed.
