05/23/2019 An unannounced inspection was begun in response to a self reported incident being received in our office. The investigation is ongoing at this time. LI was present from 7:20 AM- 9:20 AM 05/29/2019 An unannounced monitoring inspection was continued today. Based on multiple interviews, the preponderance of evidence does not support the allegation. There are no violations cited regarding this investigation. LI was present today from 8:35 AM - 9:20 AM.
