Comments:
An unannounced non-mandated inspection was completed on 05/23/2019 from 9:15 A.M.-10:45 A.M. The inspection was conducted to a response of a self-reported incident that occurred at the center in April 2019. One child's record, one staff record, attendance records, and infant daily reports were reviewed. Classroom observation and five staff were interviewed. Standard #: 22VAC40-185-370-5
Description: Based on staff interviews and review of infant daily reports, the center failed to ensure to document that 30 minutes of tummy time was provided for infants who cannot turn themselves over. Evidence: 1. It was reported to the Inspector that "tummy time" is given to all infants who require it, for at least 30 minutes daily. 2. There were eleven days between 02/04/2019-03/21/2019 that there was no documentation of tummy time for a three month old child. 3. There were nine days between 02/04/2019-03/21/2019 that there was only 10 minutes of documented tummy time for a three month old child. 4. There were two days between 02/04/2019-03/21/2019 that there was only 15 minutes of documented tummy time for a three month old child. 5. There were two days between 02/04/2019-03/21/2019 that there was only 20 minutes of documented tummy time for a three month old child.

Plan of Correction: Administration will review with all infant staff the requirement for at least 30 minutes of documented tummy time daily for infants who cannot turn themselves over.

Standard #: 22VAC40-185-570-G
Description: Based on interviews, the center failed to ensure that milk, formula, or breast milk was not to be heated or warmed directly in a microwave. Evidence: 1. Staff #4 stated "they do not put breast milk into the microwave, but that they do put formula into the microwave." 2. Administration verified that there had been formula put in the microwave to heat for the infants.

Plan of Correction: Administration will meet will all infant staff and review infant feeding and preparation requirements.
