Comments:
An unannounced monitoring inspection was conducted today from 10:00am-12:00pm. There were 25 children (3-5yrs.) supervised by 4 direct care staff. The physical plant, 2 staff records, 3 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed having circletime, storytime, observing caterpillars, and eating snack. Snacked served was cheese-itz, fruit, and water. Toileting and-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Description: Based on a review of injury reports, 4 of 5 injury reports did not contain documentation of all of the required information. Evidence: 4 of 5 injury reports did not contain documentation of the time that parents were notified.

Plan of Correction: We will remind the nurses to list the actual times that parents were notified.
