Technical Assistance:
1. Reviewed/trained vendor on new subsidy regulations. 2. Repetitive/systemic violations were found, and should be corrected to avoid further review. Completing paperwork is a requirement of the vendor agreement and regulation. 3. Forms provided: new emergency preparedness plan, and proof of identity.

Comments:
10:50am - 12:00pm Conducted an unannounced annual subsidy inspection. There are currently no children enrolled. Met with the vendor to review corrective actions from the previous inspection, and areas of regulations able to check today. Many child and adult family members were present in the home today. The daycare areas of the home were not set-up for care, nor maintained in accordance with subsidy requirements, as the vendor reported that children have not been in her care since November-December 2018. The vendor demonstrated an understanding that these areas will need to be cleaned up, organized, and safeguarded appropriately before children attend in the future. The vendor completed the annual child development training, and is currently certified in First Aid/CPR. Record keeping and safety violations found.

Description: Based on records reviewed, it was determined that 4 of 4 children's records did not include required information. Evidence: 1. Child #2 - Name, address + phone number of emergency contact person; and written authorization for emergency medical care. 2. Child #3 - Name, address + phone number of emergency contact person; and written authorization for emergency medical care. 3. Child #4 - Name, address + phone number of emergency contact person; written authorization for emergency medical care; and names of individual(s) authorized to pick the children up. 4. Child #5 - Name, address + phone number of emergency contact person; written authorization for emergency medical care; and names of individual(s) authorized to pick the children up.

Plan of Correction: These children have left enrollment [Child #2 and #3 left care in December 2018, Child #4 and #5 left care in November 2018], but moving forward the vendor will required parents to complete all required information.

Standard #: 22VAC40-665-260-A-3
Description: Based on observations made, it was determined that the hot water temperature exceeded 120 degrees. Evidence: The hot water temperature was measured to be 122 degrees today.

Plan of Correction: The hot water temperature will be turned down before children in the daycare attend.
