63.2 Facilities & Programs.
The licensing inspector conducted an unannounced monitoring inspection from 10:00am - 12:45pm. There were nineteen children in care with five staff: three infants with one staff, nine 2's and 3's with two staff, and seven preschoolers with one staff. The director was also present. The children were observed coloring, at circle time, listening to story and preparing for lunch. Five staff's and five children's records were reviewed. Standard #: 22VAC40-185-60-A Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #3) did not contain documentation that child's proof of identity and age had been verified. Plan of Correction: The director will ask the child's parent to provide proof of identity for the child's record.
