Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation, a piece of playground equipment provided was located near a concrete sidewalk. Evidence - 1. On the date of inspection (05/29/2019), the large climber/rocker red fire truck found on the back playground of the Child Day Center was located within a distance of 6 feet from a nearby sidewalk. 2. The fall zone, which encompasses a sufficient area that includes the child's trajectory in the event of a fall while the equipment is in use, would have a possible hazard for a child coming in contact with this sidewalk.

Plan of Correction: This piece of equipment shall be moved to a safe distance. In the meantime, the area shall be made off limits.

Standard #: 22VAC40-185-330-E
Description: Based on observation, the outdoor playground sandbox was not covered when not in use. Evidence - 1. On the date of inspection (05/29/2019), at approximately 11:20 am., the playground was empty with all of the children in care engaged in activities inside the center. 2. The playground sandbox was left uncovered with the tarp beside it.

Plan of Correction: Teachers shall be reminded to cover the sandbox prior to leaving the playground.
