Comments:
An unannounced monitoring inspection was conducted on May 24, 2019 from 9:40am until 11:00am. Upon arrival to the facility, there were 58 children present with 10 staff members. The children were observed participating in age appropriate activities. A sample of 5 children's records and 5 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. There were no violations cited. An exit meeting was conducted with the facility director prior to the end of the inspection.

Standard #: 22VAC40-185-280-B
Description: Based upon observation the provider did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: 1. During observation it was found that a storage area under the changing table in the two year old class room containing hazardous substances was not made inaccessible to children. a. The unlocked storage area containing hazardous substances could be a potential hazard to children. b. There were children in the two year old room during the time the violation was observed. c. The director toured the two year old room with the inspector and also identified and confirmed the violation.

Plan of Correction: The storage area was locked.

Description: Based upon observation and interview the provider did not ensure that the electrical outlets located inside of the facility had protective covers to prevent potential injuries of children. Evidence: 1. During the inspectors tour of the facility it was found that an electrical outlets located in the lunch room of the facility did not have a protective covers. a. The electrical outlets without a protective covers could be a potential hazard to children. b. There were children in the school room during the time the violation was observed. c. The director toured the lunch room with the inspector and also identified and confirmed the violation.

Plan of Correction: Covered electrical outlets with protective covers.
