Comments:
The inspector conducted an unannounced mandated inspection from 10:30 a.m. until 2:00. There were 118 children in care with staff supervising at times and maintaining ratios. Administrative staff were also present as well as kitchen staff. Seven staff records and seven children's records were reviewed. Infants and toddles were observed napping, playing, and being fed. Children were observed paying outside, playing inside, being read to, during a bathroom/washing hands break, eating lunch, and preparing for and during rest time. Medications and the corresponding paperwork were reviewed. Violations from the last inspection had been corrected. An exit interview was held with staff. The new application was reviewed. Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain documentation of additional immunizations as require for children. Evidence: 1. The inspector reviewed seven children's records. One of seven did not contain additional immunization records for child #4. The last immunization record was dated 2014.

Plan of Correction: Staff will request the documentation from the parents.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not maintain equipment in a safe condition. 1. The inspector observed three metal tricycles on the playground that were each missing a petal. 2. The inspector observed a rusty bolt with threads bare on the swing beam.

Plan of Correction: The tricycles will be removed until petals can be replaced. The bolt will be capped.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center did not maintain the required resilient surfacing on the playground. Evidence: 1. The inspector measured 2-4 inches of mulch under the swings that are a set of three. 2. The inspector observed the red slide on the infant/toddler playground that did not have the correct use zone. Slides should have 6 feet on all sides. Four feet plus the height of slide in from the slide chute. The concrete pad was in the slide's use zone.

Plan of Correction: Staff will move mulch to the proper depth. Staff may consider mats under the swings to help keep mulch from being displaced. Staff will move the slide to have the required use zone.
