Comments:
An unannounced monitoring inspection was conducted today from 7:40am-8:55am. At the begin of the inspection there was 26 children directly supervised by 2 staff. The physical plant, 2 staff training records, 5 children?s records, 1 medication, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, coloring, building blocks, and playing outside. The center was clean and organized. The inspection findings were discussed during an exit interview and the results of the inspection were emailed to the facility.Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 of 5 children's records were incomplete. Evidence: Child 3's record did not contain addresses for 2 required emergency contacts.

Plan of Correction: I will ask the parents to update the information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas inside the center were not maintained in a safe condition. Evidence: A paper cutter with a sixteen inch blade was observed upright on the floor and in the unlocked position inside of an unlocked cabinet.

Plan of Correction: We will move the paper cutter to a high shelf.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children. Evidence: An unlocked cabinet under the sink contained a canister of aerosol disinfectant spray, disinfectant wipes, and bleach spray.

Plan of Correction: We will find a way to lock the cabinet.

Standard #: 22VAC40-185-350-E-5
Description: Based on observation and staff statement, the program did not maintain a ratio of one staff member for every 18 children. Evidence: Twenty-seven children were observed with one supervising staff member from 7:56am-8:13am on the date of inspection. Staff 2 had taken children to the bus and left Staff 1 to supervise the remaining children. One staff member stated that two regularly scheduled staff members were absent on the morning of inspection.

Plan of Correction: We called in a third staff member to maintain ratio. She arrived at 8:37 on the date of inspection.

Standard #: 22VAC40-185-510-A
Description: Based on record review, prescription medication was present in the child day center without an accompanying written authorization from the child's parent and physician. Evidence: Emergency medication was present for Child 6 without written authorization.

Plan of Correction: I will ask the parents to bring an updated authorization.
