The Licensing Inspector conducted an unannounced investigation on 04/12/2019, and a follow up was conducted on 04/24/2019 in response to a self-reported incident that was received by the licensing office on 04/08/2019 regarding transportation. Based upon staff interviews, and records reviewed the preponderance of evidence gathered during this investigation does support violations related to the self-reported incident. Standard #: 22VAC40-185-580-C Description: Based on information obtained from interviews, the center transported Child#1 in a vehicle that did not contain the required emergency information. Evidence: On 04/04/19 the required emergency information and emergency telephone numbers such as emergency contact's names and telephone numbers were not in the vehicle during the time the vehicle was used to transport the child. Plan of Correction: Retrained center staff on transporting children guidelines. One staff was terminated, another received written warning.
Standard #: 22VAC40-185-580-L Description: Based on review of documentation and interviews, the facility did not obtain parental permission to transport a child before providing transportation. Evidence: On the day of the incident 04/04/19 , Staff#1 transported Child#1 to the child's school in their personal vehicle because the child missed the public school bus. Parental permission was not obtained prior to transporting the child to school. Plan of Correction: Retrained all staff and bus drivers on transportation guidelines. One staff was terminated, another received written warning.
