Technical Assistance:
LI and director discussed standard 22VAC40-185-(7)-510 regarding medication authorization.

Comments:
An unannounced renewal inspection was conducted with the director beginning at 1:25 PM and ending at 4:00 PM. There were a hundred children, ages three months through ten years old, in care during the inspection. The infants were observed playing with manipulatives and soft toys. The toddlers and preschool children were observed during laying on cots during nap time. The school-aged children were observed doing arts and crafts, playing building blocks, and playing computer games. Ten children's records were observed and in compliance. Five staff records were observed and in compliance. Five medications were reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent emergency evacuation drill was documented as May 28, 2019. The most recent shelter-in-place drill was documented as December 21, 2018. The most recent health inspection was dated January 31, 2019. The most recent fire inspection was dated September 26, 2018. Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not have all hazardous substances such as cleaning materials in a locked place using a safe locking method that prevents access by children. Evidence: There was an unlocked under the sink cabinet located in the pre-k classroom that had a bag of Voban Aromatic Absorbent, box of Borax, and spray bottle of glass surface cleaner which was accessible to children.

Plan of Correction: The cabinet was locked immediately after being found unlocked. Staff will be retrained regarding keeping hazardous substances unlocked.

Description: Based on observation, review of written attendance, and staff interview, the center did not maintain one staff member for every 18 children for children from the age of eligibility to attend public school through eight years old. Evidence: On May 29, 2019, at approximately 1:30 PM, the licensing inspector observed 41 school aged children in the care of two staff members.

Plan of Correction: The center had three staff members scheduled to be in the school age classroom to be in compliance with the staff-to-child ratio. The third staff had a personal emergency and needed to leave the center. Additional staff came in to provide supervision and bring the classroom back into ratio. The school age room was out of ratio compliance for about 10 minutes.
