Technical Assistance:
Licensing inspector discussed child abuse and neglect training with administrators.

Comments:
An unannounced monitoring inspection was conducted today from approximately 9:30am-12:30pm. There were 41 children in the direct care and supervision of 15 staff. A sample of 5 children?s records, 5 staff records, 2 medications and 5 minor injury reports were reviewed. The children were observed playing, reading, dancing, riding tricycles and engaged with staff. Children had recess indoors due to inclement weather. 1 new staff member has been hired since the last inspection. Areas of non-compliance are identified in this report. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, children's files were incomplete. Evidence: 5 of 5 children's records reviewed were missing required information. 1. Children A- E were missing previous school attended. 2. Child A was missing the mothers employer phone number. Staff A stated that the parent of child A was employed. 3. Child D was missing the employer phone number of both parents.

Plan of Correction: Staff files have been updated for the upcoming school year to reflect required information.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records were incomplete. Evidence: 5 of 5 staff records reviewed were missing required information. Staff B-F were missing the address of their emergency contact.

Plan of Correction: Emergency ccontact addresses will be added to emergency form for staff records.

Standard #: 22VAC40-185-290-3
Description: Based on observation, an electrical outlet was not covered with protective covers. Evidence: An electric outlet in Room 6 was not covered with a protective cover.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-550-M
Description: Based on record review, a minor injury report was incomplete. Evidence: 1 of 5 minor injury reports reviewed were missing required information. 1. 1 minor injury report was missing future action to prevent injury.

Plan of Correction: Program will ensure that staff completes child's injury reports entirely.
