An unannounced monitoring inspection was conducted on June 3, 2019 from approximately 9:45 am to 11:50 pm. Eight staff were present with forty-one children. Additional staff were on site for administrative and kitchen duties. Five children's records and five staff records were reviewed.

Standard #: 22VAC40-80-120-E-5 Description: Based on observation and interview, facility did not ensure a copy of the notice of the commissioner's intent to take an action was posted in a prominent place at each public entrance of the facility. Evidence: On June 3, 2019, the Notice of Intent to Deny Renewal Application for a License dated April 19, 2019 was not posted. Owner confirmed the notice was not posted. Plan of Correction: The letter was posted during inspection.
