Description: Based on an inspection of the playground, it was determined that the mulch under and around play equipment was not resilient. Evidence: The depth of the wood chips was measured in three spots around the center composite play structure on the preschool/school age playground. All three spots measured about one to two inches in depth.

Plan of Correction: Mulch has been ordered and children will be kept off of the equipment until it arrives and is in place.
