Technical Assistance:
Inspector spoke with the Director about 22VAC40-185-260 during this inspection.

Comments:
An unannounced monitoring visit and facility tour was conducted from 3:29pm-4:5pm. At the time of inspectors' arrival there were 88 children in care ranging in age from infant-preschool age with 16 staff members and 2 administrators. Children were observed participating in classroom activities, indoor and outdoor play, hand washing and transitioning between activities. A sample of 8 children's records and 8 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are detailed in the Violation Notice and were discussed with the Director at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review it was determined the center did not ensure that each child's record contained all of the required components. Evidence: The record for child #2(enrolled 2/4/19) and child #3(enrolled 2/4/19) did not contain the complete address for one of the two required emergency contacts.

Plan of Correction: Director will request this information from parent.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: 1. The changing table in room 15A and room 9 was not secured to prevent tipping and wobbled when pulled on. 2. In room 9 there was a dangling cord at child height from the radio which was placed on the window ledge. 3. In room 15A the large foam caterpillar's covering was broken exposing the foam core. 4. All of the items above presented potential injury to the children in care.

Plan of Correction: Changing tables will be anchored 6/17/19. Cord was moved during this inspection. Caterpillar was removed during this inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation it was determined the center did not ensure that all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In room 16, 13, 9 and 11 there were cleaning agents, bug spray and shaving cream in unlocked cabinets at child level. 2. All of the items above carried a "keep out of the reach of children" label.

Plan of Correction: Staff was retrained on the locking all chemicals.

Standard #: 22VAC40-185-560-G
Description: Based on observation and interview it was determined that the center did not ensure that when food is brought from home it is labeled with the date. Evidence: 1. In room 15A and 15B the children's cups were not labeled with the date. 2. The staff confirmed that these items came from home.

Plan of Correction: Center staff will ensure that all food/drinks brought from home are labeled with the child's name and date.
