Comments:
An incident was self-reported to Licensing on 3/29/19 concerning a child who was left alone in a classroom. An announced inspection was initiated on 4/4/19 jointly with a local government agency. On 4/4/19, observation of the center's video recording of 3/29/19, a review of both child and adult records was conducted and staff interviews were completed. A follow up inspection was conducted on 4/18/19. Based on the preponderance of evidence, violations were found pertaining to care and protection, staff supervision and policies. Standard #: 22VAC40-185-40-E
Description: Based on staff interviews, review of staff written statements, review of the center's policies and observation of the center's video recording system, the licensee failed to ensure compliance with the center's own policies and procedures. Evidence: l. The Center's Face to Name Policy requires that staff identify each child by face and name as each child crosses a threshold. Staff are then required to change the child's location on an electronic device. Center policy requires staff to complete a final observation of the classroom prior to leaving the classroom. 2. On 03/29/19, at approximately 5:01pm the center's video recording system showed that Staff #1 did not identify Child #1 (2yrs) by face when the class transitioned from the classroom to the outside. 3. The class then transitioned through a gate leading into the Intermediate class playground. The class was on the Intermediate class playground when Child #1 was found inside the classroom by Staff #3. 4. On 03/29/2019, Staff #1 did not complete a final observation of the classroom to ensure that no children remained in the room, leaving Child #1(2yrs) alone in the classroom for approximately 18 minutes. 5. Staff #1 entered two transitions for Child #1 on the electronic device that Child #1 had transitioned to the outside playground and also at a gate leading to the Intermediate playground area.

Plan of Correction: Staff were re-trained on face to name policy and procedures. Staff were re-trained on proper classroom and playground safety and monitoring techniques. Daily and ongoing observation of classroom transitioning and doing face to name is being done by a member of the management team.

Standard #: 22VAC40-185-340-A
Description: Based on staff interviews, and review of staff statements and the center's video recording system, two staff supervising children failed to always ensure care, protection and guidance for one child. Evidence: l. On 03/29/2019 at approximately 5:01pm, Child #1(2yrs) was left alone in the Intermediate Classroom for approximately 18 minutes while the remainder of the class went outside to the playground. 2. During the 18 minutes the child was left alone in the Intermediate class, the center's video recording of 3/29/19 shows Child #1 holding their arms outstretched and jumping up and down, pouting, sitting down on the floor with their hands over their face, and standing in front of the class perimeter door and window. 3. At approximately 5:19pm, Staff #3 was leaving for the day from the playground and noticed Child #1 alone in the class through a window. Staff #3 stated "[Child #1] was crying at the window. I told [Staff #4] immediately and they went to the classroom."

Plan of Correction: Staff were re-trained on proper classroom and playground safety and monitoring techniques. Staff were re-trained on proper protection techniques for all children.

Standard #: 22VAC40-185-340-D
Description: Based on staff interviews and staff record review, a staff member who meets the qualifications of a program leader is not regularly present in one grouping of children. Evidence: According to Staff #4 the Intermediate classroom does not have a regularly present lead teacher. Staff #1 and #2 are assigned to the Intermediate Class and do not meet qualifications of a program leader.

Plan of Correction: All staff qualifications were reviewed and all classrooms have a staff member who meets the qualifications for a program leader.

Standard #: 22VAC40-185-340-F
Description: Based on staff interviews, review of staff written statements and observation of the center's video recording system, one child was not always within actual sight and sound supervision of staff.

Evidence:
l. On 03/29/19, Child #1 (2yrs)was left alone from approximately 5:01pm to 5:19pm in the Intermediate Class after the child's class transitioned outside to the playground.
2. Both Staff #1 and #2 were not aware that Child #1 was not with them.

Plan of Correction: All staff were re-trained on the necessity of being within sight and sound supervision of children at all time. Regular and ongoing classroom observations are being completed by administration.
