Comments:
The licensing inspector conducted an unannounced complaint inspection in response to complaints received by the department on 04/05/2019 and 04/11/2019 regarding concerns of the center's administration, special care provisions, and special services. The investigation was initiated on 04/05/2019 and completed on 05/28/2019. The inspector was on site on 04/18/2019 from 10:30am to 4:00pm. Interviews with staff were completed, observations were made at the center, and documentation reviewed. The preponderance of the evidence gathered during the investigation supports part of the allegation, therefore the complaint is determined to be valid. See the violation notice for more details. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Description: Based on a review of documentation and interview, the center did not ensure that one child's medication was labeled with the required information. Evidence: 1) On 04/11/2019, the licensing inspector reviewed photographs of a bottle of a non-prescription allergy medication that was provided to the department by an outside party. The non-prescription allergy medication was labeled with Child #1's (3-year-old) name and with Child #2's (5-year-old) name. 2) During interview on 04/18/2019, Staff #1 reported that the non-prescription allergy medication that was returned to Child #1's grandparents was labeled with his name in error. Staff #1 reported Child #1 "never received the medication while at the center." Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Plan of Correction: Per the Center: To ensure that medication is labeled with the correct child's name, we will label medicine immediately upon arriving in the center. We will have another member of management review medications before placing them in a locked container. This process will be implemented by [director], center director and another member of management.
