Comments:
An unannounced monitoring inspection was conducted from 9:15 a.m. - 10:00 a.m. to investigate a self reported incident regarding supervision. Interviews were conducted, documentation, and staff records were reviewed. The self reported incident regarding supervision was determined to be valid.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on interview and review of documentation it was determined that staff did not always ensure that children remained in sight and sound supervision. Evidence: On May 2, 2019 at around 4:30 p.m. Staff A was outside on the playground supervising reportedly about 10 children. Child A (Age 3) requested to go inside to go to the bathroom. Staff A stood in the doorway of the playground and the hallway, allowing child A to go into classroom 106 (located adjacent to the playground door) to use the bathroom. Staff A reportedly became distracted by the needs of another student and stepped away from the doorway. The child was left out of sight and sound supervision for a time-frame that was unable to be determined however was reportedly less than 5 minutes. Child A was later found by their parent in the classroom.

Plan of Correction: All staff were required to attend a mandatory staff training on Thursday May 9th to review sight and sound safety protocols. We also implemented a new procedure for staff to follow when a child needs to use the restroom while outside on the playground. Staff will ask all children in their group to use the restroom prior to their outside recess times. If a child needs to use the bathroom while outside the staff assigned will page the office to come and receive the child and take them to the restroom to ensure proper sight and sound supervision. If no office staff are available at the time needed the staff have been advised to take the entire group of children into the building to allow proper sight and sound supervision of the child needing to use the restroom.
