Technical Assistance:
Make sure to conduct and document separately the lock-down and shelter in place drills.

Comments:
An unannounced monitoring inspection was conducted with the director, who was present with 30 children and four staff at the beginning of the inspection. The Licensing Inspector was able to observe circle time, graduation practice and free play. Seven children's files were reviewed today as well as 5 staff files. The center reported that there are no medication at the center at this time. Arrival time: 8:55 am Departure time: 11:20 am

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the center is not maintained in a safe condiion. Evidence; The Licensing Inspector observed at least 8 ceiling tiles in the preschool classroom that was severely warped, sagging, splitting and/or stained from water damage.

Plan of Correction: We will replace and repair the damaged tiles and address any water issues.

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that the center is not maintaining at least 6 inches of resilient surfacing underneath and surrounding the play equipment. Evidence: On the older playground the Licensing Inspector observed the following: 1) At the end of the two red slides the mulch was compacted and no longer resilient. 2) The merry- go- cycle with the 5 seat simultaneous riders was sitting on the grass and not over resilient surfacing. 3) The mulch was compacted underneath the red and the purple- merry- go rounds On the younger playground the Licensing Inspector observed the following: 1) Compacted mulch at the end of the 3 pink and one orange slides.

Plan of Correction: We will place all of this equipment off limits until we can add at least 6 inches or resilient surfacing.

Standard #: 22VAC40-185-440-L
Description: Based on observation, it was determined that a filled boppy pillow was in the crib of Infant A, who is 3 months of age.

Plan of Correction: We will not use the pillow in the crib unless we get documentation from a physician.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the Licensing Inspector observed a preschooler finish her morning breakfast and did not wash her hands or clean them with disposable wipes nor was she directed to do so by the two staff in the classroom.

Plan of Correction: We will remind the teachers to have children clean their hands after eating.
