Comments:
An unannounced Monitoring inspection was conducted in the Center on May 28, 2019. The inspection was conducted from 1:15PM to approximately 4:45PM. There were 78 children present in 6 classrooms. The ages of the children ranged from 18 months to school-age. Groups of children were observed during nap time, free play, and afternoon snack. The school-age children viewed a movie after lunch. Medications were observed in the center. The LI observed medication administration (MAT) certificates for several staff. Staff were observed interacting with the children. The center's dismissal procedures were also observed as multiple children were picked up in the late afternoon by their parent. A sample of 8 children's records and 4 staff records were reviewed. Standard #: 22VAC40-185-510-C
Description: Based on medications observed in the center, the Center failed to follow their procedures and implement methods to prevent use of outdated medication. Evidence: The LI observed medications for 4 children, prescribed emergency medications for child a. and child b. had expired in April, 2019.

Plan of Correction: The Program Director will follow up with the children's parents and will request a current refill of the medication, or based on the parent's report, the PD will research the manufacturer's web site to verify if an extension was issued for the medication due to a shortage. The medications included with the extended expiration date will be matched and identified on the medication package's date and lot number. Documentation will be maintained as applicable to verify medication's expiration date.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of staff records, the Center failed to ensure documentation of national fingerprint background checks were obtained and available for review. Evidence: Two of four reviewed staff records did not contain documentation of a national fingerprint background check. Documentation was not available for staff #2 and staff #4.

Plan of Correction: The Program Director will follow-up with Office of Background Investigations immediately to request the status of each BG check. The national fingerprint background checks will be obtained and filed in each staff member's record.
