Comments:
A Subsidy Health and Safety inspection was conducted on May 28, 2019 to review supplemental health and safety regulations. The inspection was conducted from approximately 1:15 to 4:30 PM by this Licensing Inspector. There were 78 children present, there are currently 104 children enrolled in the program. The children were observed during varied activities. Staff were interacting with the children. A sample of 12 records was reviewed which included staff and children's. Medications were observed in the center. Documentation of practiced emergency drills was reviewed. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety regulations. A violation was found during the supplemental inspection and is identified on the violation notice.

Description: Based on interview with the Program Director, the center had not developed a written policy and procedures that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.

Plan of Correction: The Center Owners and Program Director will develop a written policy and procedures defining the current day to day operations to include classroom assignment according to age and staffing schedules.
