63.2 Facilities & Programs.
An unannounced monitoring inspection was conducted on 6/4/19 from 12:25pm until 3:35pm. At the time of the tour, there were 45 children in care with 9 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed resting quietly during nap time and eating snack. Infant feeding, hand washing and restroom procedures were also observed. Transportation, medication, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Three violations in the areas of administration and physical plant are identified on the violation notice and were discussed with the center director during the exit interview.
Standard #: 22VAC40-185-60-A Description: Based on record review and interview, the center did not ensure that children's records contain all the required information. Evidence: 1. The record for child 1 does not contain the name, address or phone number for the father. 2. The center director confirmed that the record for child 1 does not contain the required information for the father. Plan of Correction: The center will have the parent fill out the missing information for child 1 at pick-up today.
Standard #: 22VAC40-185-70-A Description: Based on record review and interview, the center did not ensure that staff records contain all the required information. Evidence: 1. The record for staff 1 (date of hire: 4/17/19) does not contain the name, address and telephone number of a person to be notified in an emergency. 2. The center director confirmed that the record for staff 1 does not contain an emergency contact. Plan of Correction: The center director will have staff 1 fill out the emergency contact form today.
Standard #: 22VAC40-185-330-B Description: Based on playground observation and measurement, the center did not ensure that when playground equipment is provided, resilient surfacing shall comply with minimum safety standards. Evidence: The mulch surrounding the climbing equipment and slides on the school-age playground measured 1 inch in all areas, where 6 inches are required. Plan of Correction: A purchase order has been placed for new mulch. Until it arrives, the director will ensure the mulch currently on the playground is raked and turned over.
