An announced monitoring inspection was conducted on June 6, 2019 to review staff records. The licensing inspector reviewed administration, staff qualifications and training, background checks and licensure and registration procedures. The sample size consisted of three staff records. Areas of noncompliance are identified on the Violation Notice.

Standard #: 22VAC40-191-60-C-2 Description: Based on record review, in one of three staff records reviewed, the licensee did not deny continued employment to a staff member when the center did not have a central registry finding within 30 days of employment. Evidence: Program Coordinator verified that the hire date for Staff #2 was 3/16/19 and the Central Registry results were dated 6/6/19. Plan of Correction: We are working with HR to ensure that the proper paperwork is done post-offer.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review, in one of three staff records reviewed, the licensee did not ensure that staff submit to fingerprinting and obtain result prior to employment. Evidence: Program Coordinator verified that Staff #3 was hired on 10/28/18 and the staff member's fingerprint results were dated 11/28/18. Plan of Correction: We are working with HR to add this as a post-offer check.
