63.2(17) License & Registration Procedures
An announced record review was conducted on June 6, 2019. Licensing Inspector reviewed the areas of administration, staff qualifications/training and background checks. The sample size of records reviewed were four. The areas of non-compliance are noted on the violation notice. The results of inspection were reviewed and verified with the Recreation Program Coordinator during the exit interview.
Standard #: 22VAC40-185-160-A Description: Based on review of 4 out of 4 staff records and staff interviews, the licensee did not ensure each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: The record for staff #4 did not include a TB screening and start date of employment was 2-16-19. Plan of Correction: The staff is having the screening read tomorrow.
Standard #: 22VAC40-191-60-B Description: Based on review 4 out of 4 staff records and staff interview, the licensee did not ensure all records have an An employee or volunteer of a licensed or registered child welfare agency or of a family day home approved by a family day system must not be employed or provide volunteer service until the agency or home has the persons completed sworn statement or affirmation. Evidence: The record for staff #3, (start date September 29, 2018) had a sworn statement signed on 1-18-19. Plan of Correction: It has bee corrected and it was not done at time of hire and we are working with HR to make sure it is done before hire.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review 4 out of 4 staff records and staff interview, the licensee did not ensure all records have an Office of Background Investigations (OBI) letter stating eligibility for hire in response to the national fingerprint check prior to beginning employment. Evidence: The record for staff #3, (start date September 29, 2018) has a OBI letter of eligibility for hire dated November 9, 2018. Plan of Correction: It has bee corrected and it was not done at time of hire and we are working with HR to make sure it is done before hire.
