Comments:
An announced record review was conducted on June 6, 2019. Licensing Inspector reviewed the areas of administration, staff qualifications/training and background checks. The sample size of records reviewed were two. The areas of non-compliance are noted on the violation notice. The results of inspection were reviewed and verified with the Recreation Program Coordinator during the exit interview.

Standard #: 22VAC40-191-60-B
Description: Based on review 2 out of 2 staff records and staff interview, the licensee did not ensure all employees or volunteer of a licensed or registered child welfare agency or of a family day home approved by a family day system must not be employed or provide volunteer service until the agency or home has the persons completed sworn statement or affirmation. Evidence: The record for staff #2, (start date September 29, 2018) has a sworn statement dated October 15, 2018.

Plan of Correction: It has been corrected and we are working HR to make it a post offer check.
