Comments:
An unannounced mandated monitoring inspection was conducted today from 10:30am to 2:15pm. There were 51 children present with 12 staff. Children were observed on the playground, coloring, having tummy time, being held during bottle feedings, and napping. Reviewed 6 children's files, 6 staff files, emergency evacuation log, medications and authorizations, attendance, fire and health inspections, and physical plant. Violations:
Standard #: 22VAC40-185-210-A
Description: Based on record review, staff did not have documentation of required qualifications for the Program Leader position. Evidence: Staff being used in Program Leader position did not have required documentation verifying their qualifications.

Plan of Correction: We will get the documentation supporting their qualifications.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, are not maintained in a clean, safe and operable condition. Evidence: 1. Observed dirt filled vents in Toddler A and B room. 2. Observed a leaking toilet in the Two's bathroom 3. Observed a light fixture with visible water stains in the 2B room 4. Observed water stains on the ceiling in the Gross Motor room. 5. Observed a crack in the Toddler A sink used for children's hand washing.

Plan of Correction: I will write my DM to follow up on work orders

Standard #: 22VAC40-185-330-B
Description: Based on observation, resilient surfacing does not comply with minimum safety standards of 6 inches in depth. Evidence: The mulch on the Discovery 2's playground and Toddler Playground is less than 1 inch in depth under the slide exits, and fall zones.

Plan of Correction: We will follow up on the work order for mulch that was submitted over 6 months ago and again on Tuesday.

Standard #: 22VAC40-185-340-D
Description: Based on observation and record review, each grouping of children did not have least one staff member who meets the qualifications of a program leader. Evidence: Infant B did not have anyone in the room who was Program Leader qualified.

Plan of Correction: We will move staff to make sure there is a program leader.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, staff did not have a central registry finding within 30 days of employment. Evidence: Staff#6 (10/15/2018) did not have documentation of a central registry clearance.

Plan of Correction: It look like they started the process to tracking the finding down but it was never completed. I will resubmit a new one.
